Below are some Documents you will need in order to manage your team. If you manage/coach a T-Ball, Rookie, Minors, or Majors team, you must have every player complete the Consent for Treatment Form - Cal Ripken version. If you manage/coach a Babe Ruth team, then you must do the same using the Babe Ruth version.
